Follow these steps to restore a form that has been deleted from the Forms Management page:
- On the left navigation menu, click
Forms Management.
- In the upper-right corner of the page, click Restore Deleted Form.
- In the Restore Deleted Form dialog box, select the check boxes for the forms you want to restore.
- Click Restore, and then click Yes to confirm the restoration.
