Add Questions to a Section in AdmissionsPlus Online Forms

After you create a section, you can add questions to it by copying questions or creating new ones. See the appropriate section below to add a question using one of these methods.

Every page must have at least one section, and every section must have at least one question.

You can view the form as you create it by clicking Preview.

Copy a Question

Follow these steps to copy a question to a section:

  1. On the section, click +Add From Existing Questions.
  2. Select the check boxes for the questions you want to copy.
  3. Click Save.

    To copy multiple questions from another form, click +Add Multiple Questions, and in the Add Question dialog box, select the check boxes for the questions; then click Save.

Add a New Question

Follow these steps to add a new question to a section:

  1. On the section, click +New Question.
  2. The Add/Edit Question dialog box opens

    Select the field you want the question linked to.

    B

    Type the question as you want it to appear.  You can use the text editor to format the questions and add hyperlinks.

    C

    Select the check box to make the question mandatory.

    D

    Select the check box to make the question Read Only

    E

    Select this check box to do one of the following:

    Allow users to submit payments for additional fees. (For example, on an application form, users could submit a testing fee along with the application fee.)

    low multiple application fees for different users. (For example, you can set up different application fees for new families and returning families).

    In the Requirement drop-down list, select the requirement that is satisfied when a payment is made.

    F

    Select the type of question you want to add.

    If you select Drop Down or Radio Button, do one of the following:

    • If the field linked to the question has a field validation (meaning, the field is linked to a drop-down list of answers in AdmissionsPlus), Field Validation is automatically selected. Leave this option selected if you want to use the pre-defined drop-down list of answers.
    • If the field linked to the question does not have a field validation or if you want to provide answers in a format you define, click Answer Pool and select an answer format from the Answer Pools drop-down list.

    Click Create/View Answer Pool to add or edit an answer pool.

    G

    Type a word that is used to locate the question during searches.

    Select the requirement completed when a parent uploads a file

    Type the question as you want it to appear.  You can use the text editor to format the questions and add hyperlinks.

    C

    Select the check box to make the question mandatory.

    D

    Select who can upload attachment forms.

    E

    Type a word that is used to locate the question during searches.

    Type the instructions you want to include for the question. You can use the text editor to format the text of the instructions and add hyperlinks.

    B

    Type the instructions you want to include for the question. You can use the text editor to format the text of the instructions and add hyperlinks.


    In the Add/Edit Question dialog box under Available Questions, you can create a new question by clicking +New and designing the question settings, or by clicking an existing question and editing the settings (including the name). Click Save or Save and Add Another to save your changes.

  3. Click Save to save your changes and close the dialog box, or click Save and Add Another to save your changes and add another question.
