View Submitted Forms Log

To view a list of submitted forms, on the left navigation menu click Submitted Forms Log.

Search for a form by the first word in the form title (Begins With: Form Name) or by any word in the form title (Contains: Form Name). In Search, type the word and click .


Filter information within a column.


Export the log to Excel.


For each submitted form, view the applicant's name; form type; program, grade level, and school year being applied to, and date and time of the submission.

Export the Submitted Forms Log

Follow these steps to export the Submitted Forms Log to an Excel file:

  1. On the left navigation menu, click  Submitted Forms Log.
  2. Click Send to Excel.
  3. Click to select a Start Date and End Date for the range of dates for which you want to export submitted forms.
  4. Click Export Log.
  5. In the Save As dialog box, navigate to where you want to save the Excel file, edit the File name as needed, and then click Save
