You can activate a health questionnaire to screen students and teachers for symptoms of illness. At login, parents and teachers are prompted to complete the questionnaire and will not be able to access their portals until they submit it.

Enable the Health Questionnaire in Summary Permissions
You can choose the content of the questionnaire and the days of the week parents and teachers are required to complete it. If your school has a hybrid schedule, you can activate the questionnaire for groups and set the days of the week the questionnaire is required for group members on the Setup > Groups page.
- On the navigation bar, click Permissions.
- On the Summary tab, scroll down to Health Questionnaire.
- Select the Enable Health Questionnaire check box.
- Choose to enable the questionnaire for All Users or Group.
- If you selected All Users, select the days of the week you want parents and staff to take the questionnaire.
On the days you've selected, if the health questionnaire is active in parent and teacher permissions, they will be prompted to complete the questionnaire every time they log in to their portal until they submit it.
- To question users about symptoms, select the Section 1: Symptoms check box, and select the symptoms to include.
- To question users about potential exposure, select the Section 2: Close Contact/Potential Exposure check box, and select the options you want to include.
- Under Instructions for positive answers, type the message you want to appear on the questionnaire if a parent or teacher gives a positive response to one of the questions.
- At the bottom of the page, click Save Settings.

Activate for Parents
To activate the Health Questionnaire for parents:
- On the navigation bar, click Permissions.
- Click the Parents tab, and then scroll down to Health Questionnaire.
- Select the Active for parents check box.
- At the bottom of the page, click Save Settings.
Activate for Teachers
To activate the Health Questionnaire for teachers:
- On the navigation bar, click Permissions.
- Click the Teachers tab, and then scroll down to Health Questionnaire.
- Select the Active for teachers check box.
- At the bottom of the page, click Save Settings.
Activate an Automatic Email Alert for the Health Questionnaire and Send Reminder Notifications
You can activate an automatic email alert to notify administrators with health questionnaire permissions whenever a questionnaire is submitted that indicates a student or teacher is sick.
To activate the email alert, follow these steps:
- On the navigation bar, click Permissions.
- On the Summary tab, under Health Questionnaire, select the Enable Automated Health Questionnaire Email Alert check box.
- Once enabled, if you want to schedule a time when notifications go out, check off Schedule Notifications.
- Click , and select the time you want the notification to be sent.
- In the Time Zone drop-down list, select your time zone.
Set the Days the Health Questionnaire Is Required for a Group
If you enabled the health questionnaire for groups, follow these steps to set the days of the week a group is required to complete the questionnaire:
- On the navigation bar, click Setup > Groups.
- Next to the group whose questionnaire schedule you want to set, click .
- Click Health Questionnaire.
- Select the days of the week you want parents and teachers to complete the questionnaire.
On the days you've selected, if the health questionnaire is active in parent and teacher permissions, they will be prompted to complete the questionnaire every time they log in to their portal until they submit it.
- Click Save Group.
Generate a Report of Health Questionnaire Data
You can generate a report of users' Health Questionnaire responses or a report of who responded positively to a question or who hasn't completed the questionnaire.
- On the navigation bar, click Miscellaneous > Reports.
- In the panel on the left, click Health Questionnaire.
- For Report Type, click either of the following:
- Full Report: Generate an Excel report of users' responses to all questions, including no responses.
- Exception Report: Generate an Excel report of users who responded 'yes' to any question or did not submit the questionnaire.
If the questionnaire is enabled for groups, the Exception Report excludes users who were not required to complete the form on the chosen days.
- For Date Range, enter the date range of questionnaires you want to include in the report.
- For User Type, choose to gather the questionnaire results of All users, Students, or Staff.
- At the bottom of the page, click Generate Report.
